2. 1 piece of Passport size colored picture (Please attach on the application form) *Avoid wearing white colored top/shirt. No eyeglasses. Eyebrows and ears must be visible. No jewelries. Teeth should not be visible.

4. Photocopy of Passport Bio-page (page 2) (A4 Size Bond Paper)
5. Original & Photocopy of valid and previous visa/s and arrival stamps to OECD member countries for the past 5 years

- NO PASSPORT PULL OUT ONCE FILED. If you will need your passport while your application is at the Korean Embassy, it cannot be pulled out for whatever reason.

■ Visa Fee:
60 to 90 days stay in Korea – PHP2,000.00
More than 91 days in Korea – PHP3,000.00 (All applicants of long-term visa(90 days above) from the mentioned 35 countries will be required to submit Certificate of TB (tuberculosis) Screening issued from hospitals designated by the embassy.)
