2. 1 piece of Passport size colored picture(Please attach on theapplication form) *Avoid wearing white colored top/shirt. No eyeglasses. Eyebrows and ears must be visible. No jewelries. Teeth should not be visible.
3. Original Valid Passport with signature(should be valid for morethan 6 months) Please remove any passport covers
4. Photocopy of Passport Bio-page (page2) (A4 Size Bond Paper)
5. Original & Photocopy of valid and previous visa/s and arrivalstamps to OECD member countries for the past 5 years
a. If applicable only; Must showproof of arrival stamporslip or E-travel Pass confirmation / E-Arrival Notification

a. IF EMPLOYED
i. Original COE: Must include position, date hired, compensation, office address and a reachable contact information of signatory
ii. Photocopy of ITR (Income Tax Return) or Form 2316 Copy
b. If SELF-EMPLOYED
i. Business Registration from SEC or DTI (Copy), Business Permit or Mayor's Permit (Copy) (For Corporation attached page with list of Incorporator or latest GIS)
ii. Photocopy of ITR (Income Tax Return) or Form 2316 / 1701 with corresponding payment receipt from BIR authorized bank and
should include the pages with Net income and Total revenue
7. Spouse Bank Documents
a. Latest Original Personal Bank Certificate (Issued within 1-3 months of application)
b. Must include account name, account number, account type, current balance, account opening date
c. 6 months Average Daily Balance (ADB) must be shown
8. Original PSA Marriage Certificate for Married Female Applicants
9. COVER LETTER (If applicable)
10. Data Privacy Consent Form (Secure from Agency)
