4. SELF EMPLOYED: Mayor's Permit, DTI (Single proprietor), SEC (For Corporation, must submit front copy of SEC and GIS)
5. ITR: Copy of Latest Income Tax Return (ITR) with BIR Stamp; For Self-employed ITR must include pages with Net income and total revenue
6. BANK DOCUMENTS: Bank Certification latest issued
7. TRAVEL DETAILS: Flight Reservation/Confirmation; Hotel Confirmation; Detailed Itinerary
8. FOR FOREIGN NATIONALS: Copy of ACR/I-Card, AEP, Proof of residency or long term visa
