3. 2 pcs. good quality 2x2 colored photos (white background) *Avoid wearing white colored top/shirt. No eyeglasses. Eyebrows and ears must be visible. No jewelries. Teeth should not be visible.
4. EMPLOYED: Original Employment Certification with wet signature (Please state position, date hired, salary and contact details of HR/Signatory in Company letter head)
5. SELF EMPLOYED: Mayor's Permit, DTI (Single proprietor), SEC (For Corporation, must submit front copy of SEC and GIS)
6. ITR: Copy of Latest Income Tax Return (ITR) with BIR Stamp; For Self-employed ITR must include pages with Net income and total revenue
7. BANK DOCUMENTS: Copy of Bank Certification with Bank statements for the past 6 months (Issued within the same month of application)
8. TRAVEL DETAILS: Flight Reservation/Confirmation; Hotel Confirmation; Cruise Confirmation; Detailed Itinerary
9. PSA CERTIFICATE: Birth, Marriage Certificate (For married applicants)
