REQUIREMENTS
1. PASSPORT: Original and valid with original signature – including old passport with previous visa to Japan (if any) Must be 6months valid and have at least 2 blank pages. Please remove any passport covers
- COLORED and SCANNED COPY of FULL PASSPORT BIOPAGE
2. APPLICATION FORM: Computerized, accomplished and originally signed (A4 size paper) Should be filled out all items correctly. If item is not applicable, please fill in [N/A].
3. PHOTO: 2 pcs. colored passport size (45mm x 35mm), with white background, taken within the last 3 months. Avoid wearing white colored top/shirt. No eyeglasses. Eyebrows and ears must be visible. No jewelries. Teeth should not be visible.
Write applicant's name and birthday on back side of the photo. (Photographs that do not meet the standards or have been digitally processed or modified are not acceptable)

ORIGINAL PSA Birth Certificate and Marriage Certificate (For Female married applicants) Issued within 1 year. Exempted if applicant has old/used Japan Visa and must not have a guarantor
- Attach PSA Receipt if available, especially for PSA issued from April 2025 onwards.
- If unreadable, submit Birth/Marriage certificate issued by Local Civil Registrar (LCR).
- If Birth Certificate is “LATE REGISTRATION,” submit Baptismal Certificate and School Record (Form 137).
- If there is no record of Birth/Marriage in PSA, submit Birth Certificate issued by LCR and PSA "Certificate of No Record"

INVITERS REQUIREMENTS
a. Invitation Letter (Japan Embassy Form)
b. Documents related to the reason for the invitation (e.g., invitation letter showing the dates of the graduation ceremony, wedding, medical certificate, etc.; if the period of stay exceeds 30 days, an explanation letter is necessary.
c. Itinerary in Japan (Japan Embassy Form)
d. Statement of Service (including E-mail address of attestant, if possible)
e. Photocopy of Inviter’s passport (Pages of ID and stamps in Japan)
PROOF OF RELATIOSHIP
- If you are relatives, submit Birth Certificate or Marriage Certificate (issued by PSA within 1 year).
- If you are friends, submit photos, E-mails, receipt of remittance or parcel, etc.
IN CASE THAT APPLICANT WILL SHOULDER PART/ALL OF TRAVEL EXPENSE
a. Bank Certificate (ADB must be shown; If ADB in the last 6 months is not indicated, Bank Statement must be submitted to prove transactions within the last 6 months) ; Issued within 1 month of application
b. Copy of Income Tax Return (BIR Form)
IN CASE THAT US MILITARY PERSONNEL WILL SHOULDER PART/ALL OF TRAVEL EXPENSE
