*Picture of the applicantshouldbeattachedattheCertificateofTB(tuberculosis)Screening. The picture should bear the dry seal of the hospital or the signature of the doctor in-charge. The laboratory test result should be submitted together with the Certificate

of TB (tuberculosis) Screening.Ifresultof examinationispositivethevisaapplication is automatically denied.

- NO PASSPORT PULL OUT ONCE FILED. If you will need your passport while your application is at the Korean Embassy, it cannot be pulled out for whatever reason.

■ Processing Fees: P2,400.00
