5. Original & Photocopyofvalid and previous visa/s and arrival stamps to OECD member countries for the past 5 years

Photocopy of ACR I-Card,Philippine long-term visa, and arrival stamps to Philippines

Latest Original PersonalBank Certificate (Issued within 1-3 months of application)

i. OriginalCOE: Must include position, date hired, compensation, office address and a reachable contact information of signatory

ii. Photocopy of ITR (Income Tax Return) or Form 2316 Copy

IF SELF-EMPLOYED

iii. Business Registration from SEC or DTI (Copy), Business Permit or Mayor's Permit (Copy) (For Corporation attached page with list of Incorporator or latest GIS)

iv. Photocopy of ITR (Income Tax Return) or Form 2316 / 1701 with corresponding payment receipt from BIR authorized bank and

should include the pages with Net income and Total revenue

9. Bank Documents

a. Latest Original Personal Bank Certificate (Issued within 1-3 months of application)

b. Must include account name, account number, account type, current balance, account opening date

c. 6 months Average Daily Balance (ADB) must be shown

*Note: Applicable for Nationals who possess Philippine long-term residence visa (marriage visa, working visa, student visa, retiree's visa, permanent resident visa, and others) and have continuously stayed in the Philippines for more than 2 years will be allowed

to apply.
