8. Latest Original Personal Bank Certificate (Issued within 1-3 months of application)
a. Must include account name, account number, account type, current balance, account opening date
b. 6 months Average Daily Balance (ADB) must be shown
c. For Joint Bank accounts: Applicants name must be shown on the certificate
9. Copy of ITR (Income Tax Return) or Form 2316 with BIR Stamp
10. Original PSA Marriage Certificate for Married Female Applicants
11. COVER LETTER (If applicable)
