REQUIREMENTS
1. COPY PASSPORT with SIGNATURE: Including bio page and all used pages with amendments, endorsements, visas, and entry/exit stamps; Include all canceled or expired passports, if applicable.
2. APPLICATION FORM: Accomplished and signed forms
3. PHOTO: 2 pcs. colored passport size (45mm x 3s5mm), with white background, taken within the last 3 months.
*Avoid wearing white colored top/shirt. No eyeglasses. Eyebrows and ears must be visible. No jewelries. Teeth should not be visible.
4. EMPLOYED: Employment Certification (Please state position, date hired, salary and contact details of HR/Signatory in Company letter head); if you are a doctor, lawyer, etc., please provide a copy of your ID

5. SELF EMPLOYED: Mayor's Permit, DTI (Single proprietor), SEC (For Corporation, must submit front copy of SEC and GIS)
6. ITR: Copy of Latest Income Tax Return (ITR) with BIR Stamp; For Self-employed ITR must include pages with Net income and total revenue
7. BANK DOCUMENTS: Copy of Bank Certification with Bank statements for the past 6 months
8. TRAVEL DETAILS: Flight Reservation/Confirmation; Hotel Confirmation; Cruise Confirmation; Detailed Itinerary, Onward Visa
9. WITH INVITER / SPONSOR: Letter of support / Invitation Letter from host in New Zealand including contact details of inviter, copy of passport or valid ID
10. PSA CERTIFICATE: Birth, Marriage Certificate (For married applicants)
11. PERSONAL APPEARANCE: For Biometrics
Minor/Student Applicants: School registration or proof of enrollment, Student ID
