4. SELF EMPLOYED: Mayor's Permit, DTI (Single proprietor), SEC (For Corporation, must submit front copy of SEC and GIS)
5. ITR: Copy of Latest Income Tax Return (ITR) with BIR Stamp; For Self-employed ITR must include pages with Net income and total revenue
6. BANK DOCUMENTS: Copy of Bank Certification with Bank statements for the past 6 months; Issued within 1 month
7. WITH INVITER / SPONSOR: Letter of support / Invitation Letter from host in UK, including contact details of inviter, copy of ID/passport (if any)

b. Proof of Accommodation: Hotel reservations (Confirmed Bookings or reservations) / Proof of Mortgage or Tenancy (For invited applicants)
