Passport copies

• Copy of the signed passport biographical page and every used page showing amendments, endorsements, visas, and entry or exit stamps.
• Include copies of canceled or expired passports when applicable. Bring the original current passport to the biometrics appointment.

Application form and photograph

• Completed and signed application forms.
• One recent color photograph, 45 mm x 35 mm, with a white background, taken within the last three months. Do not wear a white top, eyeglasses, or jewelry. Eyebrows and ears should be visible and teeth should not show.

Employment or business evidence

• Employees: certificate of employment on company letterhead stating position, hiring date, salary, and HR or signatory contact details. Professionals such as doctors or lawyers should include a copy of their professional ID.
• Self-employed applicants: applicable mayor’s permit, DTI registration, SEC registration, and GIS.

Tax and financial evidence

• Latest income tax return with BIR stamp. A self-employed applicant should include pages showing net income and total revenue.
• Bank certificate and bank statements covering the previous six months.
• Savings account bankbook, time deposit certificate, and proof of remittance when applicable.

Travel arrangements

• Flight reservation or confirmation and a detailed itinerary.
• Proof of accommodation, such as confirmed hotel reservations. An invited applicant may provide proof of the host’s mortgage or tenancy.

If invited or sponsored

• Letter of support or invitation from the host in Canada, including the inviter’s contact details.
• Copy of the inviter’s passport or other valid identification.

Civil documents, biometrics, and assets

• PSA birth certificate and, when applicable, marriage certificate.
• Personal appearance for biometrics.
• Evidence of assets such as vehicle registration or land or property titles, when available.

Minor or student applicants

• School registration or proof of enrollment and student ID.
• PSA birth certificate.
• Affidavit of support and consent when not traveling with either or both parents.

Proof of relationship

• Relatives: birth and marriage certificates sufficient to establish the relationship.
• Friends, distant relatives, or non-marital partners: photographs, conversations, emails, remittance receipts, parcel receipts, or similar evidence.
