📋 Basic Requirements
Passport
- Original passport with original signature
- Must be valid for at least 6 months
- Must have at least 2 blank pages
- Include old passport with previous Japan visa (if any)
- Submit colored scanned copy of passport biopage
- Remove passport covers before submission
Application Form
- Computerized and completely accomplished
- Must be originally signed
- Printed on A4 paper
- Use N/A for non-applicable items
- All information must be accurate and complete
Photographs
- 2 pcs colored passport-size photos
- 45mm x 35mm
- White background
- Taken within the last 3 months
- No eyeglasses
- No jewelry
- Eyebrows and ears visible
- Teeth should not be visible
- Write name and birthdate at the back
PSA Documents
- Original PSA Birth Certificate
- PSA Marriage Certificate for married female applicants
- Issued within 1 year
- Attach PSA Receipt if available
- Highly recommended for PSA documents issued from April 2025 onwards
Itinerary in Japan
Accomplished Schedule of Stay using the official Japan Embassy format.
Valid ID
Copy of valid government-issued ID with original signature aside from passport.
Agency Forms
- Japan Visa Notice / Waiver
- Original Letter of Authorization
- Representative Authorization Forms
Multiple Entry Visa
Subject to Embassy approval.
📑 Special PSA Cases
- If PSA documents are unreadable, submit Birth/Marriage Certificate issued by the Local Civil Registrar (LCR).
- If Birth Certificate is marked as Late Registration, submit Baptismal Certificate and School Record (Form 137).
- If there is no PSA record, submit Birth/Marriage Certificate from LCR together with PSA Certificate of No Record.
👨💼 Additional Requirements for Employees
- Original Employment Certificate on company letterhead.

- Original Letter of Guarantee (Japan Embassy Form)
- Original Affidavit of Support (Notarized)
- Proof of Relationship between applicant and guarantor
- Birth Certificates
- Marriage Certificates
- Family Photos (if applicable)
- Bank Documents
- Employment Documents
- Business Documents (if applicable)
- Latest Income Tax Return (ITR)
- Tax Payment Certificate
📌 Financial Assessment
- Bank Certificate should be recently issued
- Financial capacity must be consistent with travel plans
- Supporting documents may be requested by the Embassy
- Source of funds must be verifiable
- Additional financial evidence may be required

Submission of complete documents does not guarantee visa approval. The Embassy of Japan may request additional documents and evaluate each application individually based on financial capability, travel purpose, employment background, and overall eligibility.
