Japan Invited By Military Personnel Based in Japan Visa for Filipinos

REQUIREMENTS

PASSPORT: Original and valid with original signature – including old passport with previous visa to Japan (if any) Must be 6months valid and have at least 2 blank pages. Please remove any passport covers
APPLICATION FORM: Original, accomplished and signed (A4 size paper) Should be filled out all items correctly. If item is not applicable, please fill in [N/A].
PHOTO: 2 pcs. colored passport size (45mm x 35mm), with white background, taken within the last 3 months. Avoid wearing white colored top/shirt.
Write applicant’s name and birthday on back side of the photo. (Photographs that do not meet the standards or have been digitally processed or modified are not acceptable)
ORIGINAL PSA Birth Certificate and Marriage Certificate (For Female married applicants) Issued within 1 year. Exempted if applicant has old/used Japan Visa and must not have a guarantor

If unreadable, submit Birth/Marriage certificate issued by Local Civil Registrar (LCR).
If Birth Certificate is “LATE REGISTRATION,” submit Baptismal Certificate and School Record (Form 137).
If there is no record of Birth/Marriage in PSA, submit Birth Certificate issued by LCR and PSA “Certificate of No Record”
